How to Schedule an Appointment

New Patients can fill out the New Patient Contact Form on this page. Patients must be a California Resident over the age of 12.

If you are an existing patient and have a specific question, you can also email the office at email@dralexoffice.com . Please save non-urgent questions or medication changes for in-person discussion during your appointment.

Refill Request Policy

-In order to receive ongoing prescriptions you must be an active patient who is following up regularly at the recommended intervals. If you have not been seen in the recommended time frame, then you will be asked to schedule an appointment.

-Before contacting us, please double-check to make sure you don't have a refill waiting for you at the pharmacy, since patients are usually given enough refills until scheduled follow ups.

-For Refill Requests please contact the office directly by email at email@dralexoffice.com , NOT your Pharmacy. Our office does not always receive automated electronic refill requests from the pharmacy so please contact us and be patient.

-In Email request include: Pharmacy Phone Number, Pharmacy Name, Your Legal Name, Date of Birth, Medication Name, Milligram dose, and Frequency taken (for example: once daily at bedtime).

-Medication requests may take a few days to fill so we recommend you keep a 7-day emergency supply in a separate bottle in order to avoid last minute requests. Please avoid requests on weekends or holidays in order to prevent delays.
